Why AI hallucinates and why the perfect answer can be dangerously wrong

You've seen it happen (or worse still, you haven't..!)



You ask an AI a question (ChatGPT, CoPilot or other - they're all as guilty as one and other) and it replies instantly with something that sounds like it came from a well read expert who also happens to be polite, structured, and suspiciously confident. It's fluent. It's tidy. It even gives you bullet points.


And then you discover one awkward detail. It is wrong.


Not a bit off. Not needs nuance. Just wrong. Invented. Confidently delivered fiction. That is what people mean when they say AI hallucinates.


The uncomfortable truth: When AI doesn't know, it predicts

Most modern AI models do not retrieve truth like a search engine. They generate text by predicting what words are likely to come next based on patterns learned from vast amounts of data.


That means the AI’s job is not be correct. Its job is produce a plausible next sentence.


If the model has enough reliable context, the plausible answer often aligns with reality. If it doesn't, it will still try to help because silence feels like failure and it will fill gaps with something that sounds right.


In other words, AI will happily give you the best answer it can generate, even when the best answer is a guess wearing a suit.


Why hallucinations happen - in layman's terms (plain English)

Here are the most common reasons:

  • Missing or unclear information. If your prompt is vague or missing key details, the model will infer. Inference can turn into invention.

  • No access to your specific reality. The AI may not have the latest data, your exact policy, your contract terms, or your medical history.

  • The confidence problem. Fluent language feels credible. The AI does not naturally sound uncertain unless it is designed and prompted to.

  • Pattern completion. If your question resembles a common pattern, the AI may output a typical process even if it does not apply to your situation.

  • Fabricated specifics. Without strict constraints, some models will invent sources, quotes, case names, or numbers because it fits the shape of an answer.


The real danger: it doesn't look wrong

A hallucination is rarely "purple penguins manage your taxes". It is more like a policy that sounds like your company’s policy, a medical explanation that sounds like a GP wrote it, a legal clause that sounds standard, or a financial strategy that sounds conservative.


It is wrong in a way that is hard to notice, especially if you are already tired, stressed, or in a rush (as we all are these days).



Doomsday scenarios: when a convincing hallucination causes real damage


Medical: the “do not worry, it is fine” tragedy

Someone has concerning symptoms and asks an AI:

“I have had chest tightness and pain down my left arm for two hours. Is it anxiety?”


The AI might produce a calm, plausible answer about panic symptoms, breathing techniques, and reassurance. It might even sound kind and caring.


But if that person should have gone to a GP or A&E, and does not, the cost can be irreversible. Even when the AI includes a disclaimer, people latch onto what they want to hear and a fluent answer can become permission to ignore risk.


Finance: “this strategy is low risk” until it isn't

Someone asks:

“How should I invest £30k for the next 12 months with minimal risk?”


The AI might suggest instruments that are inappropriate, misunderstand tax implications, miss fees, or imply protections that do not exist. It may also generalise from another country’s rules.


Because the response is neat and confident, the user thinks, "Great, sorted!". Then the market turns or the product behaves differently than described, and suddenly the minimal risk plan becomes why is my money down 18 percent.


AI didn't steal the money, but it may have nudged someone away from professional advice they would otherwise have taken.


Legal: the “sounds right” clause that wrecks a deal

A founder asks:

“Can you draft a simple contract clause to protect us if the client cancels?”


The AI produces something that looks legal-ish and uses legal-ish words, so it feels safe.


But it may conflict with local law, be unenforceable, omit key terms, accidentally grant the other party something you didn't intend, or fail to cover the scenario that actually matters.


Result: dispute, loss of revenue, reputational damage, and a painful realisation that looks like legal is not legal.


Compliance and safety: the procedure that breaks the rules

Imagine a regulated business asking AI for an internal procedure:

“What is the right way to document this incident?”


AI returns a clean process. Someone follows it. It is wrong. An audit happens.


Now you don't just have an operational issue, you have a compliance failure with a paper trail that shows you thought you were doing the right thing.


Business operations: confident nonsense that breaks production

Engineers ask AI to generate a configuration, a script, or a remediation plan. It produces something plausible. It looks correct to a tired human skimming it.


It deploys. Systems go down. Customers are locked out. A Monday morning becomes a post mortem.

How to use AI without getting burned

Treat AI like a brilliant intern, not an oracle

AI is fantastic at drafting, summarising, structuring, brainstorming, and producing options and checklists. It can accelerate work when you can verify it.


AI is risky for medical decisions, legal decisions, financial decisions, safety procedures, and anything where wrong has real consequences.


Use a simple rule: if it matters, verify it

If the output could cost you money, health, or reputation, do at least one of the following:

  • Cross check with authoritative sources

  • Ask for sources and verify them, do not just accept them

  • Consult a professional

  • Test in a safe environment before applying it to real systems


Ask better questions to reduce hallucinations

Prompts that help:

  • If you are not sure, say so.

  • List the assumptions you are making.

  • Give me 3 possible answers and what would make each true.

  • What would a cautious expert warn me about here?

  • Cite reputable sources and tell me if you cannot.


The enterprise reality: hallucinations are often a knowledge problem, not just a model problem

In organisations, AI rarely fails in isolation. It fails because it's fed messy knowledge.


If your internal content is duplicated, outdated, contradictory, scattered across tools, or missing ownership and approvals, then AI will still answer, but it will answer based on whatever it can find. That is where helpful turns into hazardous.
